Cost: $25, Cash or Check. Please make checks payable to: Foundation Sports Medicine.
Who Needs One?
- All athletes and cheerleaders who will participate at Mountain Valley or Canyon Lake High School
- New Comal ISD athletic policy dictates all athletes need a new physical every year prior to participation.
